Disenroll from homeschool?

I am 16 and want to get my GED. In the state of Kansas you are required to present a "letter of disenrollment from homeschooling". How would I make this letter? Do I just write "I am disenrolled from homeschooling" and have my mother sign it? Or do I have to do something more complicated and fancy?
